How to Request a FaxBack
First, be prepared with the following items:
- Your office’s 10-digit fax number
- The patient’s member ID or SSN
- The patient’s date of birth
Step 1:
Call Dental Select at 800-999-9789.
Step 2:
From the main prompt menu, select 2 for a provider’s office, then 2 to access the automated system.
Step 3:
Answer a few questions by using your phone’s keypad.
Step 4:
Sit back and relax. You will receive a fax at the number you provided in less than five minutes.
